# L'état de santé de mon patient s'est modifié

1. Informez votre patient qu'il doit communiquer tout changement de sa situation à l'office AI.
2. Pour soutenir votre patient, vous pouvez rédiger un nouveau rapport médical via MedAi. Connectez-vous à votre compte sur <https://portal-medai.ch/auth> et créez un nouveau rapport.
3. Dans ce rapport, détaillez :\
   • Les modifications de l'état de santé\
   • Les nouvelles limitations fonctionnelles impactant l'activité professionnelle\
   • Les changements dans la capacité de travail
4. Une fois le rapport complété, soumettez-le via MedAi. L'office AI l'utilisera pour réévaluer le droit aux prestations et adapter le projet professionnel si nécessaire.
